Access your vaccine information via MyChart

 

View your health summary

To see a summary of your medical record, including your COVID-19 vaccination information, log into your MyChart account.

Once logged into MyChart:

  • Click the "Menu" icon
  • Under "My Record," select "Health Summary"
  • Look for the "Immunizations" tab on the Health Summary page to see your immunization records

You can see your current COVID-19 Vaccine documented here, including dates of immunization.

See your past or upcoming appointments

You can view past or upcoming appointments in your MyChart account by clicking on the "Visits" link in the top navigation bar.

Click the "View After Visit Summary®" link below each past appointment to see a summary of the care you received during your visit. Your COVID-19 vaccine information, including dates of immunization, vaccine type, and lot number will be reported on this summary.

 


What are the symptoms of COVID-19?

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

What should I do if I've been exposed to COVID-19?

Whether you have been exposed to COVID-19, want to talk to a caregiver or need to make an appointment, we are here for you. Select one of the options below to get started. If you’re having a life-threatening emergency, call 911 immediately.

Schedule a video visit

Please call ahead before going to any HonorHealth location and do not go to the emergency department for testing unless you are experiencing a life-threatening emergency.


Visitor information

Please refrain from visiting patients in our medical centers if you're feeling ill, experiencing respiratory symptoms (runny nose, cough, fever), tested positive for COVID-19 or have any other respiratory infection.

Mask Policy

Masks are optional in most of our facilities,* unless you:

  • Have a suspected COVID-19 or other respiratory infection with symptoms (e.g., a runny nose, cough, sneeze).
  • Have been in close contact with or had a higher-risk exposure to someone with COVID-19 (a mask is required for 10 days after exposure).
  • Are immunocompromised.

If you meet any of the above criteria, please make sure to wear a mask that covers your nose and mouth at all times while in any HonorHealth facility. If you do not have a mask, one will be provided for you. Bandanas, neck gaiters, single-ply fabric masks, valve masks or similar face coverings are not permitted.

*Please note: Masks are still required in areas with our most vulnerable and at-risk patients, including all HonorHealth Cancer Care practice locations, or other areas clinically deemed necessary.
