With a new athletics season approaching, make sure you stay ahead of the competition by adding sports physicals to your to-do list. Remember, most school districts require a physical exam for students to participate.
Sports physicals
The HonorHealth Urgent Care team is offering sports physicals for student-athletes up through 12th grade at a reduced cost. These exams are performed by our leading-edge providers with the goal of protecting the health and well-being of our athletes and ensuring your child is able to participate in this year’s athletics season.
Reserve your spot
Reserving your spot is preferred and based on availability at each location. For questions and to reserve your spot, please call the phone number of the location nearest you.
Cost: $25. We accept cash and checks.
Prepare for your visit
- Wear a T-shirt, athletic shorts and shoes.
- Complete your medical history on the AIA Pre-Participation Physical Exam form before your reserved slot. (Please bring your completed form).
