Is it possible to live well with heart failure? Yes, and Erika’s story is proof that with an expert team and the right support, recovery is possible.

If you or a loved one is living with heart failure, you know how life-changing the diagnosis can be. When Erika came to HonorHealth, she had just received the same news. After being discharged from the hospital, Erika was quickly enrolled in HonorHealth’s Heart Failure Bridge Program, an initiative designed to help patients take charge and navigate the complex world of heart failure with education, support and expert care.

Immediately the care team welcomed Erika and created a community of support for her and her family. “The weight of her diagnosis was heavy, but from the very first moment we met Erika, we saw something remarkable begin—not just for her, but for the entire healthcare team,” says Liz Basinger, BSN, CCRN, registered nurse with HonorHealth Cardiac Rehabilitation.

The program is offered at all HonorHealth Heart Care Cardiac Rehabilitation centers and consists of eight one-hour educational sessions, designed to cover essential topics such as lifestyle changes, exercise, nutrition and medication management. “Erika’s resilience was incredibly inspiring, and our collaboration with the skilled providers from Neighborhood Outreach Access to Health (NOAH), and HonorHealth’s cardiology team was key to ensuring she received the highest quality care,” says Liz. By combining their expertise, the care team created a seamless and personalized support plan that guided Erika through her diagnosis.

“I’m grateful for the time and effort my care team took to help my family and me understand my condition. As non-native English speakers navigating healthcare for the first time, we felt empowered by the clear communication and support from every expert involved,” says Erika. “These moments of understanding have made all the difference.”

On the road to success

Erika has completed phase one of the program and is eager to dive into phase two of her recovery. She’s already scheduled a follow up with Tabitha Moe, MD, cardiologist with HonorHealth Heart Care, for an echo to check her progress. Erika’s anticipation of her results speaks volumes about her commitment and belief in the program.
