Adding a Clinical Pharmacist to your care team provides a partnership between you and your physician to help manage high-risk medications. Whether you’re on multiple meds, have diabetes, or take blood thinners, a Clinical Pharmacist will help with your specific needs:
- Review your current medications
- Provide personalized medication education

- Fingerstick test for INR if you take warfarin

A: The Clinical Pharmacist will work one-on-one with each patient to adjust medications, provide ongoing education, and help overcome challenges. You will continue to receive prescriptions from your local or mail order pharmacy. The Clinical Pharmacist has specialty training in medication management and will give special attention to the medication therapy plan that is right for you.
Q: What are some common reasons for seeing a Clinical Pharmacist?
A: Medications for diabetes, blood thinners, hospital follow-up, and other complex medication regimens.
Q: How will my doctor stay informed about my medication plan?
A: The Clinical Pharmacist will provide a detailed report to your doctor after each visit to the clinic. Your doctor can also see your medication plan in your electronic medical record.
Q: Is this a one-time visit?
A: Depending on your medication plan, some patients may need to come in for one or two visits while others will need ongoing care.
