Add your medical records to MyChart

Keeping all your medical records in one place gives you peace of mind and makes managing your health easier. With MyChart, you have two simple ways to add your records — securely sending them through a message or letting our Health Information Management team handle it for you. Choose the option that works best for you and stay in control of your care.


Option 1: Upload your records in a snap with MyChart

The fastest way to add your medical records to MyChart is by sending them through a secure message. Just follow these simple steps:

  1. Log in to your MyChart account.
  2. Go to Messages and click Send a message.
  3. Select Ask a medical question and click Next to acknowledge the disclaimer.
  4. Click Attach visit-related document.
  5. Choose the provider you want to send the document to.
  6. Add a subject and your message (limit: 500 characters).
  7. Upload your file (limit: 3 MB; accepted formats: GIF, JPG, PNG, TIFF, BMP, TIF, PDF).
  8. Click Send.

If you have questions or run into any issues with MyChart messaging, please call the HonorHealth Customer Navigation Center at 623-580-5800 and select the MyChart support option.
 

Option 2: Send a request, we’ll do the rest

The HonorHealth Health Information Management (HIM) Department can add your medical records to your MyChart account. Simply email or fax your request, along with the records, and they’ll handle the upload for you.

In your request, please include:

  • Your full name
  • Date of birth
  • Contact phone number
