MyChart proxy access

Proxy access allows you to connect your MyChart account with another person’s account. Once linked, you can view and help manage their personal health information, schedule appointments, access test results and more. It's a great way to stay informed.

Child and adolescent proxy access

For children up to the age of 18, you can request proxy access to view their MyChart account by completing the Request for MyChart Child Proxy Form (PDF).

Adult proxy access

If you are taking care of a parent or other adult, once the patient agrees to allow you to serve as their proxy, you are able to gain access through MyChart (see below) or by following the instructions on the Request for MyChart Adult Proxy Form (PDF).

How do I get access via MyChart?

You (the proxy) and the patient must both have an active MyChart account. To request access to the patient’s account from the app or website, the patient must:

  1. Log in to their MyChart account.
  2. Click Menu.
  3. Select Sharing Hub.
  4. Click A family member, close friend or caretaker.
  5. Select Manage ongoing access to your MyChart account.
  6. Fill out the information belonging to the person they wish to have access to (proxy) and submit.

How do I revoke access via MyChart?

Active MyChart users can assign and/or revoke proxy access directly through MyChart. To do so in your MyChart account, you should:

  1. Log in to your MyChart account.
  2. Click Menu.
  3. Select Sharing Hub.
  4. Click A family member, close friend or caretaker.
  5. Select Manage ongoing access to your MyChart account.
