Getting started
Thank you for your interest in our non-surgical endoscopic weight loss options. At HonorHealth, our goal is to help you live a healthier life. By considering the option to undergo an endoscopic procedure, you have taken the first step necessary to change your health and your life.

Step 1. Am I a candidate?
The Endobariatric team at HonorHealth offers minimally invasive options to help you achieve your weight loss goals. This includes procedures such as endoscopic sleeve gastroplasty, transoral outlet reduction and sleeve-in-sleeve revision.
Eligibility for each of these therapies is determined in part by what’s known as your body mass index, a numerical value of your weight in relation to your height. Obesity is defined as a BMI score of 30 or more.
Step 2. Contact your insurance
If you plan to use insurance to cover the procedure, please contact your insurance company to determine if your coverage includes weight loss benefits, or we can provide this service for you after you have completed our new patient packet.

Step 3. Complete the required new patient packet
Please download and complete our new patient packet. The information in this document will help us determine if you are a candidate for the Endobariatric Weight Loss Program.
There are several ways you can submit your completed paperwork:
- Drop off all your documents at our office. The office is open Monday through Friday from 8 a.m. to 4:30 p.m.
- Mail or fax your documents to the office.
- Email documents to ieascheduling@honorhealth.com.

Step 4. Our team will contact you
After you have sent us your completed new patient packet, along with the supporting documents, our team will contact you to answer your questions and discuss the next steps to get you started on your weight loss journey.
